Case Manager

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to clients, helping them navigate through various challenges while ensuring they receive the necessary resources and services. This role requires strong interpersonal skills, a basic understanding of financial services, and the ability to perform basic maths to assist clients effectively.

Responsibilities

  • Assess client needs and develop individualised care plans tailored to their specific circumstances.
  • Coordinate with various service providers to ensure clients receive comprehensive support.
  • Monitor client progress and adjust care plans as necessary to achieve desired outcomes.
  • Maintain accurate records of client interactions, progress, and any changes in their situations.
  • Provide emotional support and advocacy for clients, helping them understand their rights and options.
  • Facilitate workshops or informational sessions on relevant topics such as financial literacy and resource availability.
  • Collaborate with multidisciplinary teams to enhance service delivery and improve client outcomes.

Requirements

  • Proven experience in case management or a related field is preferred.
  • Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
  • Basic maths skills are essential for managing budgets and financial assessments.
  • Familiarity with financial services is advantageous, enabling effective guidance for clients in need of financial assistance.
  • Excellent organisational skills with attention to detail in record keeping and reporting.
  • Ability to work independently as well as part of a team in a fast-paced environment.
  • A compassionate approach towards helping others, demonstrating empathy and understanding in all interactions.
